How
to Request a Repeat Prescription Following discussion with your
doctor, you may be offered repeat prescriptions for long-term medication, using
a computerised system which incorporates a tear-off request form.
Once issued with one of these, please mark the items you need
on the request slip section of the prescription form, and bring it to the
surgery, or post with a stamped addressed envelope if you wish to have the
prescription posted to you.
Please allow 48 hours for us to prepare the prescription.
For non-urgent prescriptions please allow at least 48 hours to prepare the
request. For urgent requests, please be advised that this is at the doctor's
discretion and will be dealt with after the end of surgery. We are now able to
accept faxed requests to the surgery (Fax: 01483 757067), though this is at your
own risk as we cannot guarantee that we will receive the information.
Any enquiries about your medication request can be made between
8.30am-1.00pm Monday to Friday. We have a trained prescriptions administrator
who will be happy to help.

Prescription
Collection Service
If you take medication on repeat prescriptions you may wish to take advantage of
a free service offered by several of the local pharmacies. They will offer to
collect prescriptions from the surgery and have the medication ready for
collection at an agreed time at their premises. In certain special circumstances
they may also be prepared to deliver, such as to the elderly, disabled and
housebound.
If you wish to take advantage of this service you can find out which pharmacies
participate by phoning your normal pharmacist or asking at the surgery
reception.
Please complete this form and pass it to the pharmacy of your choice and then
when you need repeat medication in future all you will need to do is contact the
pharmacy. |
